ATTENTION PTA LOCAL UNIT LEADER

The registration form is available several ways. It has been mailed to current president, is available for download on the Illinois PTA website and can be filled out on the website then emailed to the state office, or filled out and faxed to the state office.

This form can, and should be, changed at anytime.  Just fill out a new form and submit, as above, when needed.  Even if you have not held elections, the Illinois PTA needs to know who will receive materials during the summer.  Please do not use the school address as this will delay your receiving materials important to your PTA.

CLICK ON THE FORM YOU WISH TO FILL OUT. NOW YOU MAY PRINT THE FORM OUT AND FILL IT OUT THE OLD FASHION WAY - WITH A PEN.  OR FILL THE FORM OUT RIGHT THERE ON YOUR COMPUTER, THEN PRINT IT OUT AND FAX IT TO THE STATE OFFICE OR FILL THE FORM OUT ON THE COMPUTER AND THEN EMAIL IT TO THE STATE OFFICE.
